Virtually all managers/directors complain that meetings take too long to accomplish too little.  To help keep meetings on track, use this form to record major decisions and any follow-up activities required.  After the meeting, circulate copies of the completed form to participants as an official record of deadline dates and of who is responsible for a particular activity.
